Waiver & Consent Agreement

Liability Waiver:  

 

I, on my own behalf, and behalf of my heirs, successors and assigns, hereby release and discharge, and agree to indemnify and hold harmless United Goal FC, Inc., and its officers, directors, members, employees, agents and representatives, its affiliated organizations and sponsors, and the owners and operators of fields and facilities used for its programs, from and against any and all liabilities, claims, damages, costs, demands or causes of action, whether known or unknown, relating to or arising out of the participation in any United Goal FC, Inc. program or activity.

 

I understand and acknowledge that dangers of personal injury are inherent in participating in soccer playing activities, including without limitation, competitive games (whether in leagues, tournaments or otherwise), training sessions, clinics and camps, and I expressly and voluntarily assume all risk of personal injury sustained in the games, training sessions, clinics and camps, including the risk of passive or active negligence of the released parties, or hidden, latent or obvious defects in any facilities or equipment used.  I acknowledge the possibility that my successors or I may not fully know the number or magnitude of all claims, and agree that this Release is a full and final release of all claims.  This Release is intended to be binding on my heirs and assigns.  This Release is being signed in consideration of the opportunity to participate in United Goal FC, Inc. programs and activities. 

 

Publicity Consent:

 

I (a) understand and agree that I may be filmed, televised, photographed, identified and may have my name, image, picture, likeness, voice, performance and biographical information (collectively, the “Image”) otherwise captured, depicted or recorded and (b) consent to the use of the Image herein by United Goal FC, Inc., and its assignees and licensees (including without limitation sponsors) at any time, for any purpose and in any manner without payment or my additional consent.
